Thank you for choosing The Gifted Fairy Concierge! Before we can begin creating a thoughtfully curated gift experience, all clients must complete our Pre-Order Questionnaire.

 
This questionnaire helps us gather important information about your recipient, preferences, budget, and gifting goals so we can provide the best possible service tailored to your needs.
 
Please Note:
• Completion of the Pre-Order Questionnaire is required before placing an order or scheduling a one-on-one consultation.
• The questionnaire must be completed fully and accurately to the best of your knowledge.
• We strongly require all clients to carefully review our Policies and Client Acknowledgement before submitting their questionnaire.
• Incomplete questionnaires may delay the consultation and ordering process.
 
Google Account Requirement
Our questionnaire is hosted through Google Forms and currently requires a Google login to access and submit.
